Abstract

Sonodynamic therapy combines ultrasound energy with chemical compounds to produce cytotoxic free radicals, which destroy cancerous tissue. A similar treatment, using light waves rather than soundwaves, has been FDA-approved for the treatment of some cancers. Aminolevulinic acid (5-ALA) is a fluorescent dye FDA-approved for tumor visualization in the brain, and preliminary studies have shown that it can be used as a sonosensitizing agent in combination with HIFU. 5-ALA, which accumulates only in tumor tissue within the brain, can be combined with HIFU to produce low temperature, chemically-based tumor ablation without the need for invasive surgery. Fluorescein is a fluorescent dye approved in Europe for tumor visualization in the brain, and preliminary studies have shown that it can be used as a sonosensitizing agent in combination with HIFU. After injection fluorescein perfuses all organs according to their vascularization. Then it rapidly washes out excepet in areas of vascular hyperperfusion or damage: infact it selectively accumulates in the tumoral interstitial spaces while the surrounding healthy tissues are cleared of it. Fluorescien can be combined with HIFU to produce low temperature, chemically-based tumor selectivce damage without the need for invasive surgery. Here, we determine the safety and feasibility of this approach to treat brain tumors in a porcine model.
